Trying to create a customizable html form for feedbacks, contact us or ordering forms? Requires to email the response to a specific email address?

Came across this free html form creator that is very customizable and highly user friendly. Free advertisement for them as I really like the idea and the ajax they used for the "customization" when creating the form.
